Ipamorelin is a synthetic growth hormone secretagogue receptor agonist, which makes it like the natural peptide hormone ghrelin. As an analogue of ghrelin, ipamorelin is a potent stimulator of growth hormone secretion. It is also among the most selective of the ghrelin mimetics, which means ipamorelin has few unintended side effects. Ipamorelin has been studied for its ability to stimulate bone formation and as a potential treatment for post-operative ileus. It is also of interest in weight loss research for its ability to modify GH secretion, impact insulin release, and alter food intake.

Ipamorelin for Weight Loss

There has been a lot of research into using ipamorelin for weight loss, but this can be confusing given the fact that ghrelin, from which ipamorelin is derived, is often referred to as the hunger hormone. If a hormone stimulates appetite, how can it possibly be useful in weight loss? The answer to this question lies in how ipamorelin stimulates appetite and how the peptide alters energy balance to favor lean body mass as opposed to fat mass. The answer also lies in the intelligent use of ipamorelin which, when paired with proper diet and conditioning, can lead to profound fat burning.

Using ipamorelin for weight loss might be more accurately referred to as using ipamorelin for promoting leaning body mass. Ipamorelin is a potent growth hormone secretagogue. And growth hormone is known for stimulating growth, hence the name! Growth hormone (GH) does not simply stimulate growth in general, but rather stimulates the growth of lean body tissues like muscle and bone. It stimulates the burning of fat. So, increasing GH hormone levels helps to build muscle and bone density while reducing adipose (fat) tissue.

Ipamorelin for Lean Body Mass

Research in mice shows that ipamorelin can increase growth hormone levels from three-fold over baseline to as much as 13-fold above baseline[1], [2]. Under such conditions, animal studies suggest that an increase of almost 9% in muscle mass and a decrease of 14% in fat mass can be expected _without_ any changes to dietary intake or exercise volume.

It is a natural consequence of aging that GH levels decline. Research suggests that GH levels at the age of 70 are 60% lower than at age 20[3]. Therefore, exercise becomes less effective as we age and why we tend to trade muscle and bone mass for fat. The older an animal gets, the greater the impact ipamorelin has in terms of body composition. Research in rats suggests that the oldest animals derive the most benefit from increases in GH secretion.

Ipamorelin Selectivity Improves Weight Loss Effects

As noted earlier, ipamorelin has few unintended side effects. This arises from the fact that, unlike other growth hormone secretagogue receptor agonists, ipamorelin has little to no effect on other hormones like thyroid hormone or cortisol[4]. This is important because cortisol promotes the degradation of lean body mass (both bone and muscle) while stimulating adipose tissue growth. Therefore, people who take large doses of steroids for inflammatory medical conditions end up with muscle wasting and weakness while gaining truncal adipose tissue.

Ipamorelin has been explored as a means of offsetting this problem with corticosteroids. It is so effective in this setting that there is interest in using ipamorelin in conjunction with corticosteroids so that dosages can be increased, and more benefits can be attained from these drugs without serious side effects[5], [6].

Ipamorelin Affects Blood Sugar Levels

One of the major problems with what most people eat today is that the foods have high glycemic loads. Most foods are quickly absorbed in the GI tract and cause large spikes in blood sugar levels. Normally, sugar is handled by insulin which helps it to enter muscle tissue and even the brain. Whatever is left over from that process is converted to fat. When the body is inundated with sugar, however, it has little choice but to produce adipose tissue as the sugar load overwhelms the ability of the pancreas to produce insulin.

Ipamorelin has been shown in animal studies to potentiate insulin release. It does this by indirect stimulation of the calcium channel on pancreatic islet cells as well as through stimulation of the adrenergic receptor pathway[7]. Boosting insulin alone would be useful in promoting the growth of lean body mass, but when combined with increased growth hormone secretion helps to explain why ipamorelin is so effective in causing fat loss and muscle growth in animal models.

Ipamorelin and Appetite Stimulation

Ghrelin analogues, like ipamorelin, are well known for their ability to stimulate hunger and therefore food intake[8]. This is another fact that appears to be counterintuitive to the ipamorelin for weight loss paradigm, but it doesn’t have to be. First, ipamorelin has the lowest levels of appetite stimulation of any of the ghrelin analogues. Second, and more importantly, ipamorelin is a ghrelin mimetic and therefore can suppress natural ghrelin release. This is important because ipamorelin can theoretically be used to manage the “hunger pangs” that ghrelin induces following weight loss. Here’s why.

Ghrelin is sometimes called an adiposity signal[9]. It tells the brain when energy stores are getting low and encourages animals to seek out food. Ghrelin is also heavily tied in with the cholinergic-dopaminergic reward system in the brain[10]. Research in rats suggests that ghrelin and endogenous opioids (e.g., endorphins, enkephalins) have somewhat opposing effects. Basically, ghrelin binds at certain opioid receptors causing discomfort (hence the term “hunger pangs”). Release of the ghrelin and binding of enkephalins causing pleasure. Thus, the displacement of ghrelin, which is accomplished by eating, leads to pleasure.

This system can theoretically be manipulated in two ways to favor weight loss. First, administration of ipamorelin at the right time can be used to thwart normal ghrelin signaling. This can be used to suppress ghrelin signaling and even to time when ghrelin signaling occurs so that it corresponds with the consumption of healthy foods as opposed to fatty or high glycemic index foods. Second, if the hunger pangs induced by ipamorelin are quelled by healthy foods, the body will naturally begin to associate these foods with pleasure. Thus, ipamorelin could theoretically be used in a coordinated way to induce the brain to crave healthy foods.

Simply put, it is important that eating be carefully regulated with ipamorelin because the peptide, while increasing GH and thus favoring lean body massive, also signals the brain to increase adiposity through the increased consumption of foods that benefit fat deposition[9]. This latter effect is independent of GH signaling and can counteract the benefits of increased growth hormone if inappropriate foods are consumed. In animal research, this is easy enough to overcome by providing fat-free foods.

As a final point, keep in mind that ipamorelin has its maximal effects approximately 40 minutes after administration in most animal models. This timeframe means that if ipamorelin is administered at the start of a meal, maximal GH secretion occurs at almost the same time that food is entering the intestine after consumption. This means that GH is at its peak at just the moment when nutrients can best be diverted toward muscle building and away from fat deposition.

Ipamorelin for Weight Loss: The Bottom Line

Research in rats suggest that ipamorelin may be an effective weight loss peptide. Achieving optimal results, however, is likely a matter of timing and discipline. Rats given a diet high in appropriate nutrients are not only more likely to build lean body mass in the setting of ipamorelin administration, but they are also more likely to build good eating habits and reward systems that will continue to maintain those benefits moving forward. Thus, using ipamorelin for weight loss might best be thought of as a means of training the body and mind to seek out optimal nutrition and make the best possible use of it. It will be interesting to see if future research conducted on the long-term effects of appropriately administered ipamorelin can demonstrate long-term changes in animal feeding behavior. If it does, ipamorelin might offer profound insight not just into basic hormone signaling, but human behavioral conditioning as well.
