Among these options are ARA 290, thymosin alpha-1, and AOD-9604, which have shown promising results in preclinical and clinical studies.

In this blog post, we will discuss the limitations of current arthritis treatments, the potential benefits of these novel therapies, and the ongoing research into their safety and efficacy. We will also explore how these treatments work and what makes them different from traditional arthritis medications.

What is Arthritis?

Arthritis is a broad term used to describe a group of conditions that affect the joints and cause inflammation, pain, and stiffness. The most common types of arthritis are osteoarthritis, rheumatoid arthritis, and gout. Osteoarthritis is the most prevalent type and typically affects older adults, while rheumatoid arthritis is an autoimmune disorder that can affect people of all ages.

There are many factors that can contribute to the development of arthritis, including genetic predisposition, age, injury or trauma to the joints, and obesity. In addition, certain lifestyle factors, such as smoking, alcohol consumption, and a sedentary lifestyle, have also been linked to an increased risk of developing arthritis.

The underlying mechanism that causes arthritis involves the breakdown of cartilage, which is the tissue that cushions the joints and allows them to move smoothly. Over time, this breakdown leads to the development of bone spurs and the loss of joint space, which can cause pain, stiffness, and inflammation.

Inflammatory arthritis, such as rheumatoid arthritis, occurs when the immune system attacks the lining of the joints, resulting in inflammation and damage. Gout, on the other hand, is caused by the buildup of uric acid crystals in the joints, leading to inflammation and severe pain.

There is no known cure for arthritis, and current treatments focus on managing symptoms and slowing the progression of the disease. Treatment options include nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroids, disease-modifying antirheumatic drugs (DMARDs), and physical therapy.

Arthritis causes are multifactorial, involving a combination of genetic, lifestyle, and environmental factors. While there is no cure, early diagnosis, and appropriate treatment like the peptides that I will be discussing below can help manage symptoms and improve quality of life.

ARA-290 and Arthritis

ARA 290 is a peptide that has shown promising potential as a treatment for arthritis. It works by activating the erythropoietin (EPO) receptor, which is involved in reducing inflammation and promoting tissue repair.

Studies have shown that ARA 290 can help reduce pain, inflammation, and joint damage in animal models of rheumatoid arthritis and osteoarthritis. In one study, ARA 290 was shown to reduce inflammation and improve mobility in rats with arthritis. Another study found that ARA 290 can promote the regeneration of cartilage tissue in vitro.

The mechanism by which ARA 290 exerts its effects on arthritis is thought to be through its ability to reduce the production of pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α) and interleukin-1 beta (IL-1β). In addition, ARA 290 has been shown to promote tissue repair by increasing the production of anti-inflammatory cytokines and growth factors, such as interleukin-10 (IL-10) and vascular endothelial growth factor (VEGF).

Furthermore, ARA 290 has been shown to have a good safety profile in clinical trials, with no serious adverse events reported.

Research Examples:

SAT0116 Safety and Efficacy of Ara290, a Non-Hematopoietic Erythropoietin Derived Peptide, in a 4 Week Phase II Open Label Trial in Patients with Active Rheumatoid Arthritis.

Background: Erythropoietin, a well-known stimulator of erythrocyte production, also has nonhematopoietic activities, that can generally be summarized as counteracting the actions of proinflammatory cytokines. It has been suggested previously that erythropoietin reduces disease activity in RA, but given the side effects of administering high doses of erythropoietin, this treatment option has not been further investigated.

Objectives: To assess the safety and efficacy of ARA290, a non-hematopoietic erythropoietin derived peptide, in active rheumatoid arthritis patients in an open-label trial.

Methods: Fifteen patients were randomized to 4 weeks of intravenous treatment with 2mg of ARA290 either once or 3x/week. The latter was the highest dosing scheme tested in healthy volunteers. Safety was assessed weekly by history taking and laboratory tests. Disease activity, using the disease activity score (DAS), functional ability, using the Health Assessment Questionnaire (HAQ), and pain on a visual analogue scale (VAS pain) were measured weekly. Paired t tests or Wilcoxon matched-pair signed-rank tests were performed where appropriate to examine the difference between baseline and after treatment.

Results: Patient groups were similar in demographics and baseline disease characteristics: median disease duration was 8 years, in 93% rheumatoid factor and in 87% anti-citrullinated protein antibodies were present, 86% had erosive disease and half of the patients had failed on ≥1 biological. Three patients experienced a non-serious infection, leading to study withdrawal in one patient. One patient (treated 3x/week) was hospitalized in week 4 for observation of subjective paranesthesia’s where no abnormalities were found. One patient, who was treated 3x/week, had a good EULAR response, one (treated 1x/week) had a moderate response, two (one from each group) achieved low disease activity without a DAS decrease >0.6 points and one (treated 3x/week) had a decrease >0.6 points but still a DAS >3.7. There was a (numerical) decrease in median VAS pain of 27 mm in the group treated 1x/week, p=0.03, and of 29 mm in the group treated 3x/week, p=0.2, and a decrease in median VAS global of 29 mm in the group treated 3x/week, p=0.03. No relevant changes in ESR and CRP or tender and swollen joint counts were observed.

Conclusions: This small open-label trial showed no major safety issues and possible (subjective and/or analgesic) efficacy of ARA290 in patients with active rheumatoid arthritis

Thymosin Alpha 1 and Arthritis

Thymosin alpha-1 is a synthetic peptide that is derived from the thymus gland. It has been shown to have a wide range of biological effects, including immune modulation and tissue repair, which make it a potential candidate for the treatment of arthritis.

Studies have shown that thymosin alpha-1 can help reduce inflammation and joint damage in animal models of arthritis. In one study, thymosin alpha-1 was shown to decrease the production of pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α) and interleukin-1 beta (IL-1β), in rats with rheumatoid arthritis. Another study found that thymosin alpha-1 can promote the regeneration of cartilage tissue in vitro.

Furthermore, thymosin alpha-1 has been shown to have a positive effect on the immune system, by increasing the number and function of T-cells and stimulating the production of other immune cells. This can help to reduce inflammation and promote tissue repair.

In clinical studies, thymosin alpha-1 has been shown to be safe and well-tolerated. In one study, thymosin alpha-1 was shown to improve joint function and reduce pain in patients with rheumatoid arthritis. Overall, the data on thymosin alpha-1 suggest that it could be a potentially effective treatment for arthritis, and further studies are needed to fully explore its therapeutic potential.

Research Example:

Thymosin-α1 expands deficient IL-10-producing regulatory B cell subsets in relapsing–remitting multiple sclerosis patients

Background: B cells are key pathogenic effectors in multiple sclerosis (MS) and several therapies have been designed to restrain B cell abnormalities by directly targeting this lymphocyte population.

Objectives: Moving from our data showing a Toll-like receptor (TLR)7-driven dysregulation of B cell response in relapsing–remitting multiple sclerosis (RRMS) and having found a low serum level of Thymosin-α1 (Tα1) in patients, we investigated whether the addition of this molecule to peripheral blood mononuclear cells (PBMCs) would influence the expansion of regulatory B cell subsets, known to dampen autoimmune inflammation.

Methods: Serum Tα1 level was measured by enzyme immunoassay. Cytokine expression was evaluated by Cytometric Bead Array (CBA), enzyme-linked immunosorbent assay (ELISA), and real-time reverse transcription polymerase chain reaction (RT-PCR). B cell subsets were analyzed by flow cytometry.

Results: Tα1 pre-treatment induces an anti-inflammatory status in TLR7-stimulated RRMS PBMC cultures, reducing the secretion of pro-inflammatory interleukin (IL)-6, IL-8, and IL-1β while significantly increasing the regulatory IL-10 and IL-35. Indeed, Tα1 treatment enhanced expansion of CD19 CD24 CD38hi transitional-immature and CD24low/negCD38hi plasma blast-like regulatory B cell subsets, which likely inhibit both interferon (IFN)-γ and IL-17 production.

Conclusion: Our study reveals a deficient ability of B cells from MS patients to differentiate into regulatory subsets and unveils a novel anti-inflammatory and repurposing potential for Tα1 in MS targeting B cell response.

AOD-9604 and Arthritis

AOD-9604 is a synthetic peptide that is derived from human growth hormone (HGH). It has been shown to have potential therapeutic effects on a range of conditions, including arthritis

Studies have shown that AOD-9604 can help reduce inflammation and joint damage in animal models of arthritis. In one study, AOD-9604 was shown to decrease the production of pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α) and interleukin-1 beta (IL-1β), in rats with rheumatoid arthritis. Another study found that AOD-9604 can promote the regeneration of cartilage tissue in vitro.

In addition to its anti-inflammatory effects, AOD-9604 has been shown to have a positive effect on the metabolism of cartilage tissue. It has been shown to stimulate the production of type II collagen, which is a key component of cartilage tissue, and to inhibit the production of enzymes that can break down cartilage.

Furthermore, AOD-9604 has been shown to be safe and well-tolerated in clinical trials. In one study, AOD-9604 was shown to improve joint function and reduce pain in patients with osteoarthritis.

Research Example:

Effect of Intra-articular Injection of AOD9604 with or without Hyaluronic Acid in Rabbit Osteoarthritis Model

Background: To investigate the effects of AOD9604 intra-articular injections with or without hyaluronic acid (HA) in a collagenase-induced knee osteoarthritis (OA) rabbit model

Design: Mature New Zealand white rabbits (n=32) were randomly administered 2 mg collagenase type II twice in each knee joint. Weekly injections of 0.6 mL saline (Group 1), 6 mg HA (Group 2), 0.25 mg AOD9604 (Group 3), and 0.25 mg AOD9604 with 6 mg HA (Group 4) were administered for 4–7 weeks after the first intra-articular collagenase injection. The degree of cartilage degeneration was assessed using morphological and histopathological findings, and the degree of lameness was observed at 8 weeks after the first collagenase injection.

Results: Mean gross morphological and histopathological scores were significantly higher in Group 1 than in Groups 2, 3, and 4, and the scores were significantly lower in Group 4 than in Groups 2 and 3. The lameness period in Group 4 was significantly shorter than those in Groups 1, 2, and 3. The lameness period in Group 1 was significantly longer than those in Groups 2 and 3. Conclusion: Intra-articular AOD9604 injections using ultrasound guidance enhanced cartilage regeneration, and combined AOD9604 and HA injections were more effective than HA or AOD9604 injections alone in the collagenase-induced knee OA rabbit model.
