Peptides Related to Weight Loss and Obesity Research

## Reestablishing Normal Physiology The answer to combatting adiposity and improving weight loss results is to reestablish normal physiological parameters. Sometimes this is referred to as “hacking your physiology” or “hacking your metabolism,” but it isn’t really hacking. All you are attempting to do is work within the confines of your biochemistry to optimize that very same biochemistry. Peptides Related to Weight Loss and Obesity Research

For decades the scientific community has insisted that the best way to lose weight is through diet and exercise. As a result, no shortage of expert advice on these topics has abounded in the form of books, videos, podcasts, exercise equipment, and fad diets. The weight loss and diet control industry in the United States alone was worth $72 billion dollars in 2019[1]. Despite the advice, Americans are heavier than ever and now they are starting to ask why.

What if the failure has nothing to do with the individuals trying to lose weight? What if the advice that the scientific establishment has provided for decades has been misguided, not though any kind of malice, but through a genuine misunderstanding of the basics of human biology? Everyone knows diet and exercise work, but until recently no one has asked _why_ they work? Furthermore, no one has asked why they worked so well a few decades ago but have become less and less effective over time. Fortunately, the answers to those questions came before they were even asked and the scientific community has started to pivot in its fundamental understanding of what _really_ governs body composition.

Peptides Related to Weight Loss and Obesity Research

Diet and exercise are the keys to healthy weight and body composition, but why does that work? Furthermore, why do both seem to become less effective and more difficult to adhere to as we age? Is our every failure to lose weight really the result of a lack of motivation and commitment or is there something more fundamental that isn’t being addressed? As the U.S. adult obesity rate has continued to climb to an all time high of nearly 43%, the answers to these questions have become more pressing than ever before. Fortunately, some almost accidental breakthroughs in our understanding of energy balance have started to provide some answers.

For a long time, doctors have insisted that energy balance was a matter of calories in versus calories out. In fact, the NIH _still_ touts this overly simplistic view of weight control[2]. The equation is simple, eat less than you burn in a day and you will lose weight. A calorie is a calorie is a calorie, right? Wrong. Not only is the “a calorie is a calorie” argument wrong, we have had the knowledge to understand _why_ it is wrong since the modern era of physiological science came along.

The newest research tells us several very important things. First, fat and cholesterol are _not_ the universal enemies we made them out to be. For some of us, fat (especially saturated fat) is more problematic than it is for others. That said, the universal culprit, which has gone almost unnoticed for decades, is the carbohydrate[3]. Looking only at raw calories ignores the specific metabolic impact that each of these basic food groups has on our metabolism.

In the case of the carbohydrate, the answer isn’t simple either. We need carbohydrates to function properly and eating the right amount of them is necessary for good health. In fact, building muscle is impossible without eating enough of the right kinds of carbohydrate. Like anything, however, the devil is in the details and how much of what type of carbohydrate we consume is critical to whether we gain or lose weight, particularly fat.

The body processes and responds to different foods in different ways. What is more, the response to a specific food also differs as a result of how fast that food is delivered to the bloodstream. If you eat a small quantity of carbohydrate that is very slowly absorbed into the bloodstream, your blood sugar will remain relatively low and so will your insulin. Your body can easily deal with this carbohydrate load and most of it will be used to fuel the brain, basic metabolism, and muscle contraction. If you eat too much carbohydrate and it is the kind that is rapidly absorbed into the bloodstream, the pancreas will respond by flooding the body with insulin[4]. At this point, auxiliary systems kick into action and, because your basic energy needs are being met, the bulk of this food energy will be socked away in storage systems. Some of it will go to glycogen in the muscle and liver, but most of this energy will be stored in fat. The problem only intensifies from here though.

The accumulation of fat is a self-reinforcing cycle in two ways. First, adipose tissue has, in recent years, been recognized not just as a storage tissue, but as an endocrine and secretory organ in its own right. The hormones released from fat, called adipokines, are now known to profoundly impact energy balance. Among those is leptin, a primary signal of satiety in the body. Fat tissue produces leptin, which one would think is a great way to reduce hunger and prevent further fat accumulation through calorie intake. Here’s the catch though. It isn’t direct leptin levels that are important so much as it is the peaks and troughs of leptin that regulate hunger. Thus, when adipose tissue produces leptin in response to feeding, it sets a new high bar for leptin levels. As soon as levels fall below this high bar, it signals the body to begin refeeding. This happens because leptin isn’t so much a satiety signal as it is a storage signal. It simply tells the body if fat stores are being created or utilized. When they are being utilized, leptin floods the body and triggers eating behavior[5]. The more fat you have, the more leptin floods your body and signals the need to eat.

The other reason that fat accumulation is self-reinforcing is that the more fat we possess, the higher our baseline levels of researched inflammation become and the greater our risk of developing metabolic syndrome. As it turns out, the researched inflammation caused by increased levels of fat suppresses the action of insulin in certain ways. The result is that the pancreas produces more and more insulin which has two effects – increasing fat accumulation and the suppression of growth hormone (GH) signaling[6].

Now we have a real problem on our hands. Not only is the body dealing with a vicious cycle of fat tissue encouraging the deposition of ever more fat tissue, but the lack of GH causes a shift as well[7]. The body now produces less muscle and bone in favor of more fat regardless of what we eat. Additionally, the lack of GH leads to decreased energy and increased fatigue. Worst of all, if you do muster the energy to exercise, the lower levels of GH mean that the work you do is less effective and your results are more modest. The problem intensifies as we age because the natural production of GH declines by approximately 10% per decade after the age of 20[8].

Now that GH levels are compromised, aging actually accelerates because growth hormone plays important roles in muscle strength, bone density, heart health, cognitive capacity, and even the health of our DNA. In the end, bad dieting advice is literally killing us. Fortunately, science is beginning to understand that once this pattern is established, the only way to undue it is to work backwards. Diet and exercise can boost GH levels to some degree, but generally not enough to overcome the deficit created by both age and increasing adiposity. The answer, at least in the animal studies to date, appears to be supplementing GH secretion to reestablish proper physiology.

Reestablishing Normal Physiology

The answer to combatting adiposity and improving weight loss research results is to reestablish normal physiological parameters. Sometimes this is referred to as “hacking your physiology” or “hacking your metabolism,” but it isn’t really hacking. All you are attempting to do is work within the confines of your biochemistry to optimize that very same biochemistry. How you go about doing this, however, depends on your current physiological status and your goals. The following is a look at both currently existing mechanisms for reestablishing physiology as well as approaches that are under scientific investigation.

What We Know About Eating for Weight Loss Research in 2024

What we know about eating has changed in recent years. Whereas science once admonished against fats and cholesterol, the winds have changed substantially. The new culprit of choice is carbohydrates, with sugar being most directly in the crosshairs. This is because the primary goal of eating should be to avoid spikes in insulin levels. The basics of healthy eating to avoid insulin spikes are covered here. For those who want to truly alter their diet for the better, a trip to a physician or a dietician (probably several, actually) is highly recommended. The basics are these.

First, cut back on sugar. There is no USDA recommendation for daily intake of sugar, but the American Heart Association recommends no more than 5 grams of added sugar per day[9]. This does not include sugars that naturally occur in foods like fruit and milk, but does include sugars in processed foods so you can’t simply stop spooning sugar into your coffee and expect a change. Instead, you will need to parse everything you eat that isn’t a raw food (e.g., fruit, vegetable, etc.). This includes looking for sugars that go by other names such as maltose, sucrose, corn syrup and more. The easiest way to identify sugars is by reading the nutritional label and looking for “added sugars” under the heading for “total sugars.”

Second, cut back on high glycemic carbohydrates. This includes things like white breads, tortillas, bagels, pie crusts, doughnuts, or anything else with flour that is easy to digest. When you eat carbohydrates, and you should, opt for less processed options to slow the rate of absorption and make massive spikes in insulin levels less common. This will help to ensure that the food you eat goes to fueling your body and not into fat storage.

Third, cut out saturated fat. Fat is fine to eat, essential in fact, but certain fats are better than others. Look for poly-unsaturated cis fats and things like omega-3 and omega-6 fats. Once again, nutrition labels are useful here, but you will also want to considered the types of fats in non-processed foods. Generally, plant-based sources of fat are healthier than animal fats. The biggest source of added saturated fat in our diets is dairy products (especially cheese).

Fourth, intermittent fasting can be useful in altering our basic biochemistry. It lowers insulin and increases the production of heat-stress proteins linked to longevity research. The most effective means of intermittent fasting appears to be time-restricted eating. By compressing all of our calorie intake into a 10-hour window, we force changes in insulin secretion patterns that can break the cycle of increasing fat and decreasing GH levels. In fact, in mouse models of 8-hour time-restricted eating, weight levels are 28% lower than in mice with 24-hour access to food[10].

If you follow the basic tips above, you will be well on the way to combating the first major problem of obesity, elevated insulin levels. Reducing sugar and carbohydrate intake will directly impact insulin levels. Reducing saturated fat intake will reduce researched inflammation and, if you are genetically susceptible, shift energy balance away from storage. Some people find that meal service companies or the use of meal replacements is a good way to get started with healthier eating. Just keep in mind as we move into the next section on exercise that you can never out-train a bad diet. No matter how much effort you put in at the gym, if the nutrition component isn’t included you will not be able to lose a significant amount of weight.

What We Know About Exercise for Weight Loss Research in 2024

If dieting is all about reducing insulin levels, then exercise is about raising growth hormone levels. For years we have been told to perform cardiovascular workouts because that increases calorie burn. However, as we just learned, simply boosting calorie burn is probably not enough. Our bodies are very efficient in their use of calories and simply burning more isn’t all that helpful because it usually still isn’t enough. What you really want to do is increase growth hormone levels and the best way to do that is with resistance training.

You don’t have to lift heavy and become shredded to benefit from the increases in GH associated with resistance exercise. Simply doing some resistance training, even body weight training, can be enough to make a huge difference. Over time, the increases in GH production will become self-reinforcing and changes in body composition will accelerate. It is hard getting started, but the long-term effects include reduce adiposity, better heart health, improved cognition, enhanced sleep, stronger bones, and even better sex.

What We Know About Peptides for Weight Loss Research in 2024

All of the above measures work, but they require dedication and motivation. Unfortunately, the declines in GH as well as the inflammation research caused by fat deposition that both come with obesity tend to make motivation difficult to muster. Even worse, when you do get some motivation, you may still not see great results because your body is less responsive to the effects of exercise thanks to low GH levels. Fortunately, science has acknowledged this conundrum and work is being done to develop solutions. Many of the peptides discussed in this section are only at the stage of animal research, but a few have entered clinical trials and a small minority are in use. Read on to see what the future holds for weight loss research.

_5-Amino-1MQ_

5-amino-1methylquinolinium inhibits NNMT signaling and leads to significant reductions in fat mass, fat cell size, and chronic inflammation in research. It is under investigation as a potential treatment for obesity and diabetes research. In mouse models, it has been found to increase GLUT-4 receptor levels in skeletal muscle and increase energy expenditure. Studies show that just 1- days after administration, mice experience a 30% reduction in fat mass[11].

_Adipotide_

Adipotide causes fat loss by causing apoptosis (cell death) in fat cells. It appears to work by cutting off the blood supply specifically to fat tissue[12]. Adipotide entered phase 1 clinical trials in 2011. Interestingly, primate models have shown a decrease in food consumption following researched weight loss with adipotide. This is likely because of the reduction in leptin troughs as discussed earlier.

_AICAR_

AICAR (5-aminoimidazole-4-carboxamide ribonucleoside) is s short peptide that regulates insulin receptors and how muscle cell’s function. Research in mice shows that AICAR reduces researched inflammation in adipose tissue and thus reduces insulin resistance[13]. It also increases the number of GLUT-4 receptors on muscle tissue. Administration of AICAR appears to have similar long-term benefits to exercise[14].

_AOD9604_

AOD9604 is a modified fragment of growth hormone. In mouse models, it has been shown to increase fat metabolism and lower insulin levels. It has been tested in Phase 2b clinical trials in which it was shown to triple rates of researched weight loss compared to placebo[15].

_Cardarine_

Cardarine increase energy expenditure by increasing the amount of energy available to cells. Research in mice shows that Cardarine increases exercise capacity, allowing mice to run a full 100 minutes longer with Cardarine than without[16]. This results in increased exercise performance and increased fatty acid metabolism.

_Epithalon/Epitalon_

Epithalon can improve sleep-wake cycles to restore balance to circadian rhythms. While not directly related to weight loss research, Epithalon’s effects on sleep research do improve overall body composition. This is because GH release is the highest during certain sleep research stages. By improving the efficiency of sleep in research, epithalon increases the efficiency of GH release and can help combat obesity. Another peptide that has a similar mechanism is delta sleep-inducing peptide (DSIP).

_Growth Hormone Releasing Hormone Analogues_

There are a handful of peptides that mimic the effects of growth hormone releasing hormone to increase GH levels and thus improve muscle building, bone strength, and fat burning. These peptides include CJC-1295, Fragment 176-191, GRF, Modified-GRF, and sermorelin. These peptides have also been shown to increase wound healing, reduce the effects of aging, improve heart health, and even directly boost the health of DNA. A great deal of research is currently be invested into GHRH analogues, particularly sermorelin, as they may offer a solution to the decline in GH levels that occurs with age.

Tesamorelin, also called Egrifta, is the only growth hormone releasing hormone analogue currently FDA approved for human use. This particular peptide is currently _only_ used to treat HIV-associated lipodystrophy, but is under investigation for the treatment of adiposity in general. Research shows that tesamorelin can reduce adiposity by upwards of 20% in its target population[17]. Research also shows that it is 4 times more effective than all other anti-adiposity therapies combined[18].

_Growth Hormone Secretagogue (Ghrelin) Analogues_

A handful of peptides mimic the effect of ghrelin to increase GH levels. These peptides include GHRP-2, GHRP-6, Hexarelin, and Ipamorelin. Stimulation of the growth hormone secretagogue receptor, aka the ghrelin receptor, has been researched in the treatment of cancer cachexia, age-related cognitive decline, diabetes research, and obesity[19]. The receptor and the peptides that act on it are hot research topics currently.

_Liraglutide_

Liraglutide is a GLP-1 analogue that can lower blood sugar levels by enhancing insulin secretion. It also slows gastric emptying and reduces intestinal motility. Both of these features slow the rate of absorption of carbohydrates from the GI tract. They also increase feelings of fullness and help to curb appetite[20].

_Melanotan and Melanotan 2_

Though melanotan and its derivatives (melanotan 2, PT-141, Bremelanotide) are primarily known for their ability to increase sexual arousal, animal research also shows that they are capable of altering energy balance. These peptides work through the melanocortin system to decrease energy intake and boost energy expenditure. Research shows that melanotan 2 administration can reduce calorie intake by as much as 400 kcal/day[21].

_MOTS-c_

While not directly a peptide that induces researched weight loss, MOTS-c nonetheless belongs on this list because of its ability to impact energy balance. MOTS-c works at the level of mitochondria to help these energy factories of the body make better use of raw materials. Research in rats shows that MOTS-c increases exercise capacity and reduces metabolic stress[22].

_Semaglutide_

Semaglutide is a GLP-1 analogue developed to lower blood sugar levels by enhancing insulin secretion. Semaglutide also delays gastric emptying and reduces intestinal motility, leading to increased fullness, decreased appetite, and few spikes in blood sugar levels[23], [24].

Peptide Summary

As you can see, the peptides listed above target each of the various areas of abnormal physiology that lead to weight gain. Some peptides target insulin resistance and blood sugar levels, others directly target adipose tissue and the researched inflammation it causes, and still others target growth hormone deficiency. Some, like the growth hormone releasing hormone analogues, target all three aspects of abnormal physiology. While many of these peptides are still in animal research stages, a handful have entered into clinical trials as ongoing research continues to uncover their properties.

Weight Loss and Obesity Research in 2024

As we learn more about the biochemical processes that control our bodies, it becomes clearer that diet and exercise work not entirely because they alter our energy balance, but because they trigger certain biochemical changes that alter energy balance. Research is revealing that it is possible to trigger those biochemical changes in other ways, including peptides. While diet and exercise remain staples of a healthy, long-term solution to weight management, scientists are working to make directly accessing biochemical pathways a reality so that we can overcome some of the detrimental cycles that are all-too-quickly established when our physiology starts to go off the rails.
